Dr. Nihalani points out: “If they have not done a proper job documenting and they're coming back to the primary care physician, they don’t know what they did, so now they're in the dark.”

Financially, the stakes are equally high. Inaccurate or incomplete documentation can trigger insurance audits, lead to “clawbacks,” or even attract penalties from medical boards. “There’s a lot that can happen when the documentation tools don't work like they're supposed to,” he warns.

The Future: AI in Healthcare Documentation

But it’s not all doom and gloom. Both Medina and Dr. Nihalani are optimistic about the transformative potential of AI in healthcare. New “ambient scribe” technologies can listen passively to clinical encounters and generate accurate, editable notes for provider review. Dr. Nihalani has started to use such tools in his own practice, finding that they “drastically reduce the time required for documentation” without sacrificing accuracy or detail.

He believes the most valuable AI solutions are the ones that “disappear into the workflow”, meaning they enhance care without being intrusive or disruptive. Rather than adding to the cognitive load, these tools serve up relevant insights and clinical decision support in real time, without requiring extra clicks or distracting alerts.

What Should Clinicians and Health Systems Demand?

When shopping for the right documentation tool, Dr. Nihalani says that usability should be non-negotiable: “Design and usability… would be number one.” AI and documentation solutions must integrate smoothly, accurately extract key details, and - crucially - allow clinicians to focus on patient interaction, not paperwork.

Feedback from other clinicians underscores the potential. Dr. Nihalani was especially gratified when a nurse practitioner, unaware of his involvement in Inferscience, told him, “It’s really helped my life,” underscoring that these efforts are making a tangible difference.
